The Sky Lakes Medical Center Inc. seeks a skilled Licensed Practical Nurse to support patient care and discharge readiness in our onsite unit. You will perform focused assessments, document findings, and collaborate with RNs and the care team to implement care plans.

Qualifications

• Must hold Oregon State LPN licensure.
• Proficiency in basic nursing skills, patient assessment, and documentation.
• Knowledge of discharge planning processes and basic patient education techniques.
• Strong communication and interpersonal skills; ability to work collaboratively within a multidisciplinary team.
• Flexibility and adaptability in a dynamic healthcare environment focused on discharge readiness.
• Preferred: prior experience in acute or transitional care settings.

Responsibilities

• Perform focused assessment of the patient’s priority condition under the RN’s plan of care.
• Develop and implement the focused plan of care as identified by the RN.
• Provide direct patient care and document patient status and responses.
• Evaluate patient progress toward expected outcomes and adjust as needed with the RN.
• Communicate patient status and concerns to the RN and care team.
• Assign and supervise tasks within licensed scope; coordinate with CNA/UAP as needed.
• Document assessments, interventions, and progress in the electronic health record.
• Assist with discharge planning and reinforce patient/family education.
