Job summary

Memorial Health is seeking a Patient Discharge Specialist to support discharge planning and coordination. You will verify insurance benefits, discuss post-discharge options with patients and families, and arrange required documentation and hand-offs to facilities or home health services.

The role requires an active LPN license and a minimum of two years in acute or long-term care, along with strong communication and office computer skills to coordinate care and ensure timely, compliant discharge

Qualifications

• Strong oral and written communication skills required.
• Customer service experience and/or training preferred.
• Office computer skills required.

Responsibilities

• Discharge Coordination: Coordinate discharge plans with Patient Care Facilitator/Social Work staff and discuss options for complex patients.
• Interact with patients/families to discuss discharge preferences and facility/home health options; refer and coordinate accordingly.
• Coordinate transportation needs and ensure smooth hand-offs to facilities or home health services.
• Maintain updates on patient status with nursing facilities and home health agencies; arrange first home health visit.
• Complete Medicare/Medicaid documentation and ensure timely follow-up with providers and payors.
• Assist in data collection for department objectives and support process improvements.
• Participate in on-call and weekend coverage as required.
