• General computer skills: data entry, word processing, email, records management.
• Demonstrates critical thinking skills.
• Effective written and verbal communication skills.
• Ability to work independently and with others.

Responsibilities

• Provides skilled nursing care to patients as ordered by physician and under RN supervision.
• Implements orders for medications and treatments as ordered by physician and in accordance with nursing care plan.
• Provides information to RN to assist in developing individualized plan of care.
• Evaluates effectiveness of interventions and seeks guidance for redirection when necessary.
• Reports to supervisor on patient care rendered and patient needs.
• Identifies patient problems and contributes to interventions.
• Assists in identification of learning needs and reinforces self-care.
• Designates applicable charges to patients using hospital system.
• Maintains environment conducive to safety and adheres to safety protocols.
• Performs other duties to support organizational effectiveness.

The Licensed Practical Nurse is responsible for providing general nursing care to patients, under the direction of a Registered Nurse and in accordance with physician’s orders and established standards and protocols. Essential Functions

• Provides skilled nursing care to patients as ordered by the physician and under the supervision of a Registered Nurse.
• Implements orders for medications and treatments as ordered by the physician and in accordance with nursing care plan as written by the RN.
• Provides information to the Registered Nurse to assist in the development of an individualized plan of care for each patient.
• Participates in evaluation of the effectiveness of interventions in relation to patient responses and seeks guidance in redirection when necessary.
• Reports to supervisor on patient care rendered, problems concerning patients’ needs, and resolution to those needs.
• Helps identify patient problems and potential risks and contributes to the development of interventions.
• Assists in identification of patient’s learning needs and reinforces self-care and other techniques.
• Appropriately designates applicable charges to individual patients using the hospital’s established system.
• May be required to work during inclement weather and other staffing emergencies.
• Provides an environment conducive to safety for patients, visitors, and staff.
• Assesses the risks for safety and implements appropriate precautions.
• Complies with appropriate and approved safety and Infection Prevention standards.
• Maintains regular attendance in keeping with assigned work schedule.
• Performs other duties as assigned to support overall effectiveness of the organization.

Minimum Job Requirements

Minimum Education & Experience : Two years’ experience in inpatient hospital setting, inpatient rehabilitation experience preferred.

Required Licenses, Certifications, and/or Documentation

• Current state license as a Licensed Practical Nurse required.
• Current BLS certification required.

Required Knowledge, Skills, and Abilities

• Knowledge of and adherence to current nursing theory and practice and infection prevention standards of practice.
• Knowledge of accreditation standards to ensure adherence to all standards set forth by state and accrediting agencies of TJC and CMS.
• Knowledge of clinical operations and procedures, and utilizes proper policies, procedures, and guidelines in caring for patients.
• Demonstrates general computer skills including data entry, word processing, email, and records management.
• Demonstrates critical thinking skills.
• Effective organizational and time management skills.
• Effective written and verbal communication skills.
• Ability to prioritize, meet deadlines, and complete complex tasks.

Physical Requirements Over the Course of a Shift

• A significant amount of standing, walking, bending, reaching, lifting, pushing, and pulling, often for prolonged periods of time.
• Both gross and precise motor functions.
• Lifting/exerting of up to 50 lbs.
• Possible exposure to bodily fluids.
• Visual acuity required for patient assessment and documentation of care.
• Acute hearing required for accurate patient assessment.
• Sufficient manual dexterity to operate equipment and computer keyboard.
• Close vision and the ability to adjust focus.
