Legacy Health is seeking a licensed practical nurse (LPN) to provide patient-centered care on a long-term care unit. You will collaborate with RNs, CNAs, and other staff to assess, plan, implement, and evaluate care for medically complex patients.

Qualifications

• Graduate of an accredited school of nursing.
• Current applicable state Licensed Practical Nurse (LPN) required.
• AHA BLS for Healthcare Providers

Responsibilities

• Contributes to and communicates age-appropriate admission and daily bio/psycho/social assessments; collects, reports and records data accurately.
• Observes patient condition or changes in condition.
• Documents observations timely, legibly and concisely per guidelines.
• Validates patient classification indicator via documentation.
• Contributes to nursing diagnosis and clinical problem identification.
• Identifies and reports significant deviations from the norm to the RN, MD and oncoming shift.
• Participates in establishing and updating individualized plan of care with RN, patient and family.
• Provides data and identifies priorities; involves patient/family in care planning.
• Implements planned nursing and medical interventions under supervision; adheres to standards of practice.
• Evaluates patient responses and contributes to care plan modifications; communicates progress to RN.
