Job summary

Monarch Healthcare Management is seeking an LPN to provide direct, quality resident care under the supervision of the RN in Minnesota. You will participate in developing and modifying the resident Plan of Care and ensure interventions are carried out as ordered by the RN or physician.

You will collect health data, report changes, and supervise unlicensed staff to ensure high-quality care and safety for residents, while upholding the BON standards and resident rights.

Qualifications

• Experience in skilled nursing or assisted living facilities is required.
• Current MN LPN license and CPR certification are required.
• Completion of MN Board of Nursing approved nursing program or equivalent is required.

Responsibilities

• Provide direct routine resident care under RN supervision.
• Collect health data and report changes to the RN.
• Supervise unlicensed staff and support plan of care implementation.
