Job summary

HarborChase of Madison seeks an attentive RN to deliver direct, individualized nursing care to residents under the supervision of the Director of Resident Care. You will follow physician orders, implement care plans, and monitor residents for changes.

You will ensure accurate documentation in the MAR, assist with self-care activities, and participate in safety and quality initiatives. The role requires teamwork, strong communication with families, and timely reporting of emergencies.

Qualifications

• Provides direct and individualized nursing care under supervision.
• Administers medications per physician orders and documents appropriately.
• Assesses resident needs and communicates plan of care to families.

Responsibilities

• Ensures quality and safe delivery of health services to residents
• Implements plan of care formulated by physician and DRC
• Provides accurate and timely documentation of care given
• Assesses and provides resident and family information pertinent to diagnosis or plan of care
• Administers medication per physician orders and documents on MAR
• Assembles resident charts and checks forms for accuracy
• Prepares incident/accident reports and notifies physician per policy
• Maintains a safe working environment and follows clinical standards
• Assists resident in learning appropriate self-care activities
• Organizes resources to meet resident needs and responds to emergencies
• Maintains confidentiality and resident rights
