HarborChase, LLC. is seeking a licensed practical nurse/licensed vocational nurse to provide direct nursing care to residents in a Florida long-term care setting.

You will follow physician-ordered plans, document care, and support residents and families in understanding diagnoses and treatments. The role requires at least 1 year of nursing experience in a health care facility, familiarity with EMR/documentation, and a compassionate approach to senior care.

Formación

• Graduated from an accredited program for LPN/LVN and is currently registered in the state
• Must obtain Continuing Education credits as required by the state
• Training in Alzheimer's and Dementia with focus on Validation Therapy
• Minimum of one year nursing experience in a health care facility
• Experience in a long term care community and/or geriatric nursing facility preferred
• Experience in emergency care and first-aid
• Able to communicate effectively with all levels of management, associates, residents, family members, and outside contacts
• Experience with Microsoft Office and Outlook software preferred
• Able to make independent decisions
• Willing to incorporate new methods into existing practices
• Passion to work with and around senior citizens

Responsabilidades

• Provides direct and individual nursing care to assigned residents
• Implements plan of care formulated by physician and DRC
• Documents care accurately and timely
• Assesses residents and provides information to residents and families
• Administers medications per physician orders and documents on MAR
• Maintains resident charts and forms accuracy
• Prepares incident/accident reports and notifies physician and family as policy dictates
• Maintains safe and healthy working environment
• Assists residents in self-care activities
• Organizes resources for resident care needs
• Recognizes emergencies and initiates actions
• Maintains confidentiality of resident information
• Performs other duties as assigned
• Demonstrates safety and ownership of associate and resident safety
• Discloses any medication that might impair performance
