Qualifications

• Active LPN/LVN license in Illinois.

Responsibilities

• Provides direct and individualized nursing care to residents under supervision of DRC.
• Implements physician and DRC plan of care.
• Documents care accurately and timely (MAR).
• Assesses residents and communicates information to care team.
• Administers medications per physician orders and monitors reactions.
• Maintains resident charts and completes required forms.
• Prepares incident/accident reports and notifies appropriate parties.
• Ensures a safe, clean working environment and adheres to standards.
• Supports residents in learning self-care activities.
• Leads safe operation and appropriate resource use for resident care.
