Job summary

HarborChase, LLC. is seeking a qualified nurse to provide direct and individualized care to residents in our community. You will administer medications, implement physician and Director of Resident Care plans, and document care accurately on the MAR.

You will assess residents and provide essential information to families and caregivers, while maintaining a safe environment and complying with state, federal, and HRA standards. Additional duties as assigned by supervision may apply.

Qualifications

• Provides direct and individual nursing care to assigned residents.
• Administers medication per physician orders and documents on MAR.
• Implements plan of care formulated by physician and DRC.
• Assesses and provides resident and family/care giver information pertinent to diagnosis or plan of care.

Responsibilities

• Ensures quality and safe delivery of health services to residents.
• Assembles resident charts and ensures all necessary forms are complete and accurate.
• Prepares incident/accident reports for residents per policy.
• Maintains safe and healthy working environment and uses safe work methods per clinical standards.
• Performs any other duties requested by supervisor.
• Recognizes emergencies and life-threatening situations and initiates appropriate action.
