Job summary

Landmark of Itasca in Itasca, IL seeks a compassionate Licensed Practical Nurse (LPN) to deliver direct patient care, supervise nursing assistants, and uphold quality care for residents.

You will collaborate with an interdisciplinary team to develop and implement individualized care plans, perform nursing procedures, and maintain accurate documentation, while adhering to facility policies and CQI initiatives.

Qualifications

• Current, unencumbered LPN license in Illinois.
• Minimum of 1 year of LPN experience.
• Strong communication and interpersonal skills.
• Ability to work in a fast-paced environment with interruptions.
• Ability to work evenings, weekends, and holidays.

Responsibilities

• Provide direct patient care to residents.
• Supervise and mentor nursing assistants.
• Develop and implement individualized care plans with the care team.
• Perform nursing procedures including medications, wound care, and basic life support.
• Maintain accurate patient care records and communicate with providers.
• Participate in CQI initiatives to improve outcomes and reduce readmissions.
