Lakeview Health Center & Assisted Living in Wisconsin is seeking an experienced Licensed Practical Nurse (LPN) to provide high‑quality resident care in a long‑term care setting under RN supervision. You will administer medications, support care planning, and help ensure a safe, respectful, person‑centered environment.

You will supervise Nursing Assistants, train staff, document changes, and participate in ongoing education to maintain care standards and resident rights.

Qualifications

• Current LPN license in Wisconsin.
• Graduation from an accredited nursing program.
• Long‑term care experience preferred.
• Knowledge of medication administration and nursing standards.
• Strong communication and teamwork skills.
• Ability to supervise and coordinate Nursing Assistant assignments.
• Must pass background check per Wisconsin Caregiver Law.
• Valid Wisconsin driver’s license if driving is required.

Responsibilities

• Provide nursing care within the Nurse Practice Act.
• Administer medications and treatments under RN supervision.
• Assist with training and orientation of new staff.
• Observe, document, and report resident conditions and changes.
• Support development and implementation of individualized care plans.
• Assign and oversee work of Nursing Assistants.
• Prepare shift reports and communicate updates to supervisors.
• Maintain resident rights, confidentiality, and safety.
• Participate in required in‑service education.
• Promote person‑centered care and positive resident interactions.
