Job summary

InterCommunity, Inc. in Hartford, CT seeks a Licensed Practical Nurse to provide evidence-based nursing care in Withdrawal Management and Intensive units, contributing to whole person healthcare.

You will administer medications, document vital signs, coordinate with the healthcare team, and support patients through transitions of care; CT LPN license and BLS/CPR required; EPIC experience preferred.

Qualifications

• Current Connecticut LPN license
• Basic Life Support (BLS)/ CPR certification
• EPIC experience preferred, but not required

Responsibilities

• Timely administer medications to patients and monitor patients for reactions or side effects
• Maintain accurate, detailed reports and records
• Record patients' medical information and vital signs
• Monitor, record, and report symptoms or changes in patients' conditions
• Receive and transcribe written orders from the medical and psychiatric provider
• Medication Reconciliation process
