Job summary

InterCommunity is seeking a Licensed Practical Nurse (LPN) to join our team in Connecticut. The role supports withdrawal management and intensive units, with responsibilities for medication administration, patient monitoring, and coordination with the healthcare team.

We require an active CT LPN license and BLS/CPR certification. EPIC experience is preferred; candidates should demonstrate initiative, strong time management, and excellent communication and organizational skills.

Qualifications

• Current Connecticut LPN license is required.

Responsibilities

• Timely administer medications and monitor for reactions or side effects.

• Receive and transcribe orders from medical/psychiatric providers; coordinate with care team.
• Direct or supervise residential technicians during shifts.
• Attend mandatory department/agency meetings and trainings.

Summary

The Licensed Practical Nurse (LPN) contributes to whole person healthcare by providing evidenced-based nursing skills to care for patients in both the Withdrawal Management and Intensive units.

Essential Duties & Responsibilities

• Timely administer medications to patients and monitor patients for reactions or side effects
• Maintain accurate, detailed reports and records
• Record patients' medical information and vital signs.
• Monitor, record, and report symptoms or changes in patients' conditions.
• Receive and transcribe written orders from the medical and psychiatric provider
• Medication Reconciliation process
• Facilitate effective crisis intervention
• Assist the RN in consulting and coordinating with healthcare team members to assess, plan, implement, or evaluate patient care plans to include referrals to other levels of care when appropriate
• Assist the RN in modification of the patient treatment plans as indicated by patients' responses and conditions and collaborate within a multidisciplinary team to achieve best patient outcomes
• Direct or supervise residential technicians while on shift
• Provide medical re-education to patients as needed
• Performs related duties as required.
• All agency staff are required to attend all mandatory department/agency meetings and trainings*

1st Shift: 7:00 AM - 730 PM

Week 1: Tuesday, Wednesday, Saturday

Week 2: Sunday, Thursday, Friday
