Job summary

Dodge County Hospital’s New Directions unit seeks an experienced LPN to provide direct and indirect patient care in a geriatric behavioral health setting. The role supports psychosocial, physical, and general care needs under RN supervision and collaborates with the treatment team to ensure patient safety and treatment plan adherence.

Responsibilities include nursing assessments, med administration with rights, documentation, and family education within scope of practice.

Qualifications

  • Licensed Practical Nurse (LPN) with current license.
  • Geriatric or behavioral health experience preferred.
  • BLS certification preferred.
  • Ability to work under RN supervision and follow policies.

Responsibilities

  • Performs patient care under the direct supervision of a Registered Nurse.
  • Performs nursing assessments and reassessments per policy.
  • Administers medications following the five rights to reduce errors.
  • Updates and revises the plan of care based on patient response.
  • Documents care accurately and coordinates with an interdisciplinary team.

Job description

LPN (Days) (PRN)

New Directions is the Geriatric Behavioral Health Unit for Dodge County Hospital. The role includes direct and indirect patient care services that meet psychosocial, physical, and general care needs, communicate with patients and families, and work under the supervision of a Registered Nurse.

Hours

Days; shifts vary and will be discussed in the interview.

Responsibilities
  • Performs patient care responsibilities considering needs specific to the standard of care for the geriatric patient and the patient's clinical condition.
  • Demonstrates the ability, under the direct supervision of a RN, to perform nursing assessments on all patients and reassessments as per policy.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Demonstrates knowledge of medications including psychotropic drugs and their correct administration based on the patient's clinical condition.
  • Follows the five medication rights and reduces the potential for medication errors.
  • Demonstrates ability to revise the plan of care as indicated by the patient's response to treatment and evaluate overall plan daily for effectiveness.
  • Formulates and implements patient treatment plans; family is included when appropriate.
  • Demonstrates ability to perform treatments and provide services to level of licensure.
  • Communicates appropriately and clearly to team leader, coworkers, and physicians.
  • Consents with members of the treatment team for the benefit of the patient.
  • Consults other departments, as appropriate, to provide for an interdisciplinary approach to the patient's needs.
  • Provides nursing care appropriate to the mental and physical condition of the geriatric patient.
  • Documentation meets current standards and policies.
  • Maintains and operates equipment safely and correctly.
  • Treats patients and families with respect and dignity; provides emotional support.
  • Provides education to the patient and family concerning treatment plans, compliance with the treatment plan, medications, and their side effects within scope of practice.
  • Coordinates and supervises patient care under the direct supervision of a Registered Nurse.
  • Demonstrates an ability to be flexible, organized and function under stressful situations.
  • Assists with research studies conducted in the department.
  • Performs other duties as assigned within the LVN/LPN scope of practice.
Skills
  • Proven experience as LPN or similar role.
  • Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team.
  • Familiarity with hospital health, safety and sanitation standards and procedures.
  • Understanding of confidentiality obligations and nursing best practices.
  • Excellent knowledge of medical and hospital terminology.
  • Good knowledge of MS Office and data entry.
  • Excellent communication and interpersonal skills.
  • Compassionate and able to handle stress.
License, Registrations, Certifications

BLS Certification (Preferred).
LPN LICENSE (Required).
