Job summary

Dodge County Hospital is seeking an LPN for the Day shift in the New Directions Geriatric Behavioral Health Unit. This full-time role involves direct and indirect patient care, under the supervision of a Registered Nurse, with emphasis on geriatric psychosocial needs and safety.

Responsibilities include nursing assessments, medication administration, plan of care revisions, and collaboration with physicians and the treatment team.

Qualifications

  • LPN license required and BLS preferred.
  • Experience in geriatric or behavioral health settings preferred.
  • Ability to collaborate with RNs and physicians.
  • Commitment to patient safety and confidentiality.
  • Strong communication and documentation skills.

Responsibilities

  • Provide direct and indirect patient care for geriatric patients.
  • Under RN supervision, perform nursing assessments and reassessments.
  • Ensure patient safety and reduce medical errors.
  • Administer psychotropic medications appropriately.
  • Follow medication rights and monitor effects.
  • Update care plans and involve family as appropriate.
  • Coordinate with the treatment team and other departments.
  • Document according to standards and maintain equipment.
  • Educate patient and family about treatment and medications.
  • Supervise care under RN and adapt to stressful situations.
  • Assist with research studies in department.
  • Perform other duties within LVN/LPN scope.

LPN (Day) (Full-Time) Please Note: New Directions is the Geriatric Behavioral Health Unit for Dodge County Hospital.

HOURS: Days, 7am-7pm

Provides direct and indirect patient care services that meet the psychosocial, physical and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a Registered Nurse. Communicates with physicians and team members about changes in patients' conditions, including results of diagnostic studies and symptomatology. Ability to respond quickly and accurately to changes in condition or response to treatment.

Responsibilities
  • Performs patient care responsibilities considering needs specific to the standard of care for the geriatric patient and the patient's clinical condition.
  • Demonstrates the ability, under the direct supervision of a RN, to perform nursing assessments on all patients and reassessments as per policy.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Demonstrates knowledge of medications including psychotropic drugs and their correct administration based on the patient's clinical condition.
  • Follows the five medication rights and reduces the potential for medication errors.
  • Demonstrates ability to revise the plan of care as indicated by the patient's response to treatment and evaluate overall plan daily for effectiveness.
  • Formulates and implements patient treatment plans; family is included when appropriate.
  • Demonstrates ability to perform treatments and provide services to level of licensure.
  • Communicates appropriately and clearly to team leader, coworkers, and physicians.
  • Concurs with members of the treatment team for the benefit of the patient.
  • Consults other departments, as appropriate, to provide for an interdisciplinary approach to the patient's needs.
  • Provides nursing care appropriate to the mental and physical condition of the geriatric patient.
  • Documentation meets current standards and policies.
  • Maintains and operates equipment safely and correctly.
  • Treats patients and families with respect and dignity; provides emotional support.
  • Provides education to the patient and family concerning treatment plans, compliance with the treatment plan, medications, and their side effects within scope of practice.
  • Coordinates and supervises patient care under the direct supervision of a Registered Nurse.
  • Demonstrates an ability to be flexible, organized and function under stressful situations.
  • Assists with research studies conducted in the department.
  • Performs other duties as assigned within the LVN/LPN scope of practice.
Skills
  • Proven experience as LPN or similar role.
  • Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team.
  • Familiarity with hospital health, safety and sanitation standards and procedures.
  • Understanding of confidentiality obligations and nursing best practices.
  • Excellent knowledge of medical and hospital terminology.
  • Good knowledge of MS Office and data entry.
  • Excellent communication and interpersonal skills.
  • Compassionate and able to handle stress.
