Job summary

REM Community Services, a part of Sevita, is hiring for a full-time nursing position. You will provide prescribed medical treatment and personal care under the direction of an RN or physician, maintain patient health records, and coordinate with external agencies as needed.

The role requires an associate degree in nursing or a state nursing certificate, one year of relevant experience, and current LPN/LVN licensure. CPR/FA and related certifications are expected, with travel as needed.

Qualifications

• Associates degree in nursing or state nursing certificate.
• One year of experience in nursing with the population served.
• Current LPN/LVN licensure for the state of residence.
• CPR/FA and HIV/AIDS certification, BBP, and ACT training per state regulations.

Responsibilities

• Provides basic care and medical treatment under RN/physician direction.
• Monitors medications and inputs health diagnoses for care plans.
• Administers medications and oversees staff who administer meds.
• Coordinates with external agencies and maintains health care records.
• Trains staff on medication administration, infection control, and safety protocols.
• Travels as needed for service delivery.
