Job summary

REM Community Services, part of the Sevita family, is hiring a Licensed Practical Nurse (LPN) to provide nursing services and supports to individuals with complex needs. Daytime first shift with full-time hours.

Qualifications

Nursing degree or state certificate required.
At least one year of nursing experience with population served.
Current LPN/LVN license in state of residence.
Valid driver’s license, car registration, and auto insurance.
CPR/FA, HIV/AIDS, BBP, and ACT training as required by state regulations.
Strong communication with staff, individuals served, and families.

Responsibilities

Monitor health conditions of individuals served and ensure quality services.
Administer medications and provide documentation; oversee staff administering medications.
Provide prescribed medical treatment per physician’s orders and personal care services.
Conduct health assessments and implement care plans in ISP; respond to condition changes.
Provide staff training on precautions, infection control, and medication administration.
Liaise with medical and professional providers; coordinate services with external agencies.
