Job summary

Sevita, through ResCare Community Living, provides nursing services to individuals with disabilities in home and community settings. We are seeking an LPN/LVN to join our team 2pm-10pm with rotating days, offering $35/hour and a full benefits package.

You will monitor health, administer medications, document care, and support ISP plans. Requirements include an LPN/LVN license in state, at least one year of nursing experience with the population served, and CPR/FA certification.

Qualifications

Associate’s Degree in Nursing or related field required.
One year of nursing experience with population served required.
Current LPN/LVN license in state of residence required.
CPR/FA and HIV/AIDS certification, BBP, ACT training as per state licensing requirements.
Ability to communicate effectively with staff, individuals served, and families.

Responsibilities

Monitor health conditions and ensure quality services.
Administer medications and document appropriately; oversee medication administration by staff.
Provide prescribed medical treatment and personal care services.
Assess and implement health care services in each ISP; respond to condition changes.
Provide staff training on precautions, infection control, and medication administration.
Coordinate with medical professionals and external agencies to ensure consistent care.
Educate individuals served on self-management and health topics.
