Job summary

REM Community Services, part of Sevita, is seeking a Licensed Practical Nurse to provide nursing services and supports in a community-based setting in Minneapolis, MN. This role offers varied, rewarding challenges and the opportunity to make a meaningful difference in the lives of individuals with complex medical and behavioral needs.

You will monitor health conditions, administer medications, document treatments, conduct assessments for ISP plans, and train staff on universal precautions,

Qualifications

• Associate’s Degree in Nursing or related field.
• One year of nursing experience with the target population.
• Current LPN/LVN license in the state.
• Valid driver’s license, car registration, and auto insurance.
• CPR/FA and additional state certifications as required.
• Strong communication with staff, individuals served, and families.

Responsibilities

• Monitor health conditions and ensure quality services are provided.
• Administer medications and document treatments; supervise staff during med administration.
• Provide medical treatments per physician orders and personal care.
• Conduct ISP-related health assessments and respond to condition changes.
• Train staff on universal precautions, infection control, and medication administration.
• Liaise with other medical and professional service providers.
• Educate individuals served on self-medication, treatments, and health topics.
