Sevita is seeking a Licensed Practical Nurse/Licensed Vocational Nurse to provide specialized nursing services in a home/community setting. You will care for individuals with various needs, administer medications, document care, and support staff in a dynamic team.

You’ll assess health changes, coordinate ISP-driven care, and train teammates on safety protocols, infection control, and medication routines, while advocating for high-quality, person-centered services.

Qualifications

• Associate degree or state nursing certificate required.
• One year of nursing experience with the served population.
• Current LPN or LVN licensure for the state of residence.
• Valid driver’s license, car registration and auto insurance.
• CPR/FA, HIV/AIDS, BBP, and ACT training as required by state regs.

Responsabilités

• Monitor health conditions and ensure quality services.
• Administer medications and document; supervise medication staff.
• Provide treatments per physician orders and personal care services.
• Assess needs and implement health care services in ISP.
• Train staff on precautions, infection control, and medication administration.
• Liaise with medical providers and external agencies for coordinated care.
• Educate individuals served on self-medication, treatments, sex ed, and smoking cessation.
