Job summary

Peak Rehabilitation Hospital in Apex, NC is seeking an experienced Licensed Practical Nurse (LPN) to join our inpatient rehabilitation team. You will provide direct patient care, administer medications, support rehabilitation goals, and collaborate with an interdisciplinary team to optimize recovery and independence for our patients.

The role requires current LPN licensure, CPR/BLS, and prefer IV certification within 6 months. Prior Medical/Surgical experience in acute care is valued.

Qualifications

• Must hold current/active LPN Certification and graduate of an accredited LPN program.
• CPR/BLS certification is required.
• IV Certification strongly preferred or must be obtained within 6 months of hire.
• Minimum six months of Medical/Surgical experience in an acute care setting preferred.
• Strong clinical, assessment, and patient care skills.
• Effective communication and patient education abilities.
• Ability to work collaboratively in an interdisciplinary team.

Responsibilities

• Administers medications safely and accurately.
• Follows proper medication administration procedures to reduce risk of errors.
• Assists the RN with patient assessments and reassessments in accordance with policy.
• Identifies and implements appropriate rehabilitation nursing interventions.
• Completes timely and accurate Quality Improvement (QI) assessments under RN supervision.
• Delivers patient care based on treatment response and updates care plans as needed.
• Assesses and manages patient pain using appropriate interventions.
• Educates patients and families on treatment plans and recovery expectations.
• Promotes patient safety and helps reduce the risk of medical errors.
• Develops individualized patient and family education plans.
