Job summary

Signature HealthCARE of North Hardin is hiring a full-time LPN/LVN for day shifts with 12-hour rotations and weekend rotation. A $7,500 sign-on bonus is offered, along with a comprehensive benefits package

We seek a dedicated nurse to provide direct patient care, supervise staff, and maintain up-to-date documentation within our resident-centered care model. CPR/BLS may be preferred and prior long-term care experience is desired.

Qualifications

• Current, active LPN/LVN license in the state of practice (Kentucky).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role is preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Supervise nursing assistants and medication aides during shifts per policy.
• Monitor residents' status and communicate changes to physicians or families.
• Develop, implement, and review individualized care plans with the care team.
• Complete accurate clinical documentation and coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and staff education.

How you Will make a difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
