Job summary

Signature HealthCARE is seeking a licensed LPN/LVN to join our Georgetown (KY) nursing team for full-time day shifts. You will provide direct nursing care, administer medications, supervise aides, and help residents maintain safety, comfort, and quality of life.

Qualifications

Current, active LPN/LVN license in the state of practice.
CPR/BLS certification preferred.
Minimum one year of clinical nursing experience; long-term care experience preferred.
Experience in a supervisory or charge nurse role preferred.
Strong clinical observation, medication administration, and documentation skills.
Knowledge of Federal and State long-term care regulations.

Responsibilities

Provide direct nursing care to residents, including medication administration and wound care.
Assist residents with activities of daily living (ADLs).
Supervise nursing assistants and medication aides during shifts.
Monitor residents’ status and notify physician or responsible party of changes.
Develop and review individualized resident care plans with the team.
Complete clinical documentation per policy and procedure.
