Job summary

Millington Healthcare is seeking a Licensed Practical Nurse (LPN) to join our team of experienced clinicians. The role involves directing Nursing Assistants, planning discharge services, and ensuring accurate documentation of patient care in a long-term care environment.

The ideal candidate holds a current LPN license in the state of employment and has familiarity with long-term or post-acute care settings.

Qualifications

• Must hold a current and valid LPN license in the state employed.
• Long term care/post-acute care setting experience preferred.

Responsibilities

• Direct the day-to-day functions of Nursing Assistants under current rules and guidelines.
• Assist in planning residents' discharge services as required.
• Complete accident/incident reports as necessary.
• Chart nurses’ notes reflecting care provided and residents’ responses.
• Prepare and administer medications as ordered by physician in accordance with regulations and policy.
• Order prescribed medications, supplies and equipment as needed.
• Ensure narcotics handling adheres to regulations and maintain narcotic records.
• Make daily rounds to ensure staff perform duties to standard.
• Notify attending physicians and next-of-kin when conditions change or incidents occur.
• Attend and participate in center in-service programs as required.
• Complete daily nursing evaluations and documentation in Point Click Care.

Responsibilities:

• Direct the day-to-day functions of the Nursing Assistants in accordance with current rules, regulations, and guidelines that govern the long-term care center.
• Assist in planning the nursing services portion of the resident’s discharge plans as required.
• Complete accident/incident reports as necessary
• Chart nurses’ notes in an informative and descriptive manner that reflect the care provided to the residents, as well as the residents’ responses to the care
• Prepare and administer medications as ordered by the physician in accordance with federal, state, and local regulations and as required by center policy
• Order prescribed medications, supplies, and equipment as necessary and in accordance with established policies
• Ensure that narcotics are handled in accordance with federal, state, and local regulations and as required by center policy and that narcotic records are accurate for your shift
• Make daily rounds of your unit/shift to ensure that nursing service personnel are performing their work assignments in accordance with acceptable nursing standards
• Notify the residents’ attending physicians and next-of-kin when there is a change in the residents’ conditions or if the residents are involved in an incident or accident
• Attend and participate in all center in-service programs as required
• Daily Required Nursing evaluations and documentation is completed in Point Click Care.

Qualifications:

• MUST hold a current and valid LPN license in the state employed
