Job summary

Topaz Fiscal Services seeks a Licensed Practical Nurse (LPN) to join our team for 12-hour shifts with a $3,500 sign-on bonus. We are hiring 4 nurses for 7A-7P and 2 nurses for 7P-7A to support residents in our long-term care community.

Responsibilities include directing nursing assistants, assisting in discharge planning, medication administration, narcotics handling, and daily rounds. Valid LPN license in the state is required; LTC/post-acute experience is preferred.

Qualifications

• MUST hold a current and valid LPN license in the state employed.
• Long term care/post-acute care setting preferred.

Responsibilities

• Direct the day-to-day functions of the Nursing Assistants in accordance with current rules and guidelines governing the long-term care center.
• Assist in planning the nursing services portion of the resident’s discharge plans as required.
• Complete accident/incident reports as necessary.
• Chart nurses’ notes reflecting the care provided and residents’ responses.
• Prepare and administer medications as ordered by the physician in accordance with regulations and center policy.
• Order prescribed medications, supplies, and equipment as necessary per policies.
• Ensure narcotics are handled per regulations and records are accurate for the shift.
• Make daily rounds to ensure nursing staff perform assignments to standard.
• Notify attending physicians and next-of-kin when resident conditions change or incidents occur.
• Attend and participate in center in-service programs as required.
• Daily nursing evaluations and documentation in Point Click Care.

• Chart nurses’ notes in an informative and descriptive manner that reflect the care provided to the residents, as well as the residents’ responses to the care
• Prepare and administer medications as ordered by the physician in accordance with federal, state, and local regulations and as required by center policy
• Order prescribed medications, supplies, and equipment as necessary and in accordance with established policies
• Ensure that narcotics are handled in accordance with federal, state, and local regulations and as required by center policy and that narcotic records are accurate for your shift
• Make daily rounds of your unit/shift to ensure that nursing service personnel are performing their work assignments in accordance with acceptable nursing standards
• Notify the residents’ attending physicians and next-of-kin when there is a change in the residents’ conditions or if the residents are involved in an incident or accident
• Attend and participate in all center in-service programs as required
• Daily Required Nursing evaluations and documentation is completed in Point Click Care.
