Job summary

Armor Health is seeking a Licensed Practical Nurse (LPN) to join our Corrections Center of NW Ohio team in Stryker, OH. This role provides holistic nursing care in a correctional setting and supports physicians, RNs, and allied health staff.

You will perform examinations, document patient data in the EMR, administer TB tests, obtain histories, and ensure appropriate care while coordinating transfers and documenting changes using the POMR format. A current LPN license and CPR are required.

Qualifications

• Current/Active state license as an LPN
• Current CPR certificate
• Proficient with electronic medical records
• Ability to pass pre-employment Level 2 clearance to include a background check and drug screen

Responsibilities

• Provide holistic nursing care in a correctional setting.
• Assist health care providers, RNs, and allied health professionals with examinations, treatments, special tests, and routine procedures.
• Obtain patient health history, laboratory specimens, vital signs and TB tests.
• Conduct lab tests and ensure results are delivered to the correct physician or RN.
• Ensure patient is receiving proper care for injury or condition.
• Review patient chart; records and reports observed symptoms, reactions, treatments, and changes in patient's conditions.
• Coordinate patient transfers to medical facilities or outside health care providers; administer medications; implement orders; document contacts using POMR.
