Health Advocates Network, Inc. seeks a Licensed Practical Nurse (LPN) in Gig Harbor, WA to deliver direct patient care in a correctional setting.

The role involves medication administration, vital signs monitoring, and contributing to individualized care plans under RN supervision. Applicants must hold a WA LPN license, have at least 1 year of recent LPN experience, and provide PSV documentation along with BLS, N95 fit testing and immunization records.

Qualifications

• Current, valid Washington LPN license in good standing.
• Minimum of 1 year of LPN experience within the last 3 years.
• PSV (Primary Source Verification) from the licensing board or equivalent.
• BLS, N95 fit test, immunization records, TB test and related compliance documents.

Responsibilities

• Provide direct patient care under supervision of a RN or physician.
• Administer medications and treatments as prescribed.
• Monitor and record vital signs; report changes promptly.
• Assist with assessments and implement care plans.
• Perform wound care and specimen collection as needed.
• Document care in EMR accurately.
• Educate patients and families on health maintenance and plans.
• Assist with admissions, discharges, and transfers.
• Maintain a safe, clean, therapeutic environment; follow policies.

Shift Details:

• Tuesday-Saturday 6.00am-02.30pm
• Wednesday-Sunday 02.00pm-10.30pm

Licensed Practical Nurse (LPN) Duties & Responsibilities:

• Provide direct patient care under the supervision of a Registered Nurse or physician.
• Administer medications and treatments as prescribed.
• Monitor and record patients' vital signs and report changes in condition.
• Assist with patient assessments and implement individualized care plans.
• Perform wound care, specimen collection, and other basic nursing procedures.
• Document patient care accurately in the electronic medical record (EMR).
• Educate patients and families on health maintenance and treatment plans.
• Assist with admissions, discharges, and transfers.
• Maintain a safe, clean, and therapeutic environment for patients.
• Follow facility policies, infection control protocols, and regulatory standards.

Minimum Qualifications

• Current, valid, and unrestricted Washington license/certification in good standing, as required for the position.
• Minimum of 1 year of recent experience as a LPN within the last 3 years.
• Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.
• Copy of all current required certifications and compliance documents (e.g., . BLS, N95 fit test, immunization records, TB test,...).
