Job summary

Tidewell Hospice, part of Empath Health, invites an LPN to join our continuous care team on 12-hour night shifts (8:00 PM – 8:00 AM) across southwest Florida. You will provide bedside care under RN supervision, report changes, and support patients and families in times of crisis.

The role requires a Florida LPN license and a nursing program credential, plus at least one year of acute care experience; hospice experience is preferred. Join us in delivering Full Life Care.

Qualifications

• Graduate of an approved nursing program with Florida LPN license.
• Minimum one year in acute care, direct patient care; hospice LPN experience preferred.

Responsibilities

• Assist with physician orders to maximize patient comfort; support overnight triage when needed.

Schedule

1:1 Patient Ratio | Three 12-hour shifts per week (8:00 PM – 8:00 AM), with alternating weekends (specific days to be determined).

Coverage Area

Manatee, Charlotte, DeSoto, and Sarasota counties.

What You'll Do

Under physician’s orders and RN supervision, the Continuous Care (CC) Licensed Practical Nurse (LPN) is responsible for the observation of patient/family needs and providing supportive care and emotional support to the patient/family unit in times of crisis in order to maximize patient comfort and enhance quality of life.

• Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.
• In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.
• Continuous Care LPNs also assist Triage and Care Centers overnight when Continuous Care cases are pending.

What You'll Need

• Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
