Job summary

Tidewell Hospice, part of Empath Health, seeks a Licensed Practical Nurse (LPN) to join the care team in Sarasota, FL. Under physician orders and RN supervision, you’ll provide bedside care, support patients and families, and help maintain comfort and quality of life for individuals with advanced illness.

Requirements include graduation from an approved nursing program, a current Florida LPN license, and at least one year of acute care experience; hospice LPN experience is preferred.

Qualifications

• Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
• Minimum one year experience in an acute care setting, providing direct patient care; hospice LPN experience is preferred.

Under physician’s orders and RN supervision, the Continuous Care (CC) Licensed Practical Nurse (LPN) is responsible for the observation of patient/family needs and providing supportive care and emotional support to the patient/family unit in times of crisis in order to maximize patient comfort and enhance quality of life.

Primary tasks, duties, and responsibilities:

1. Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.

2. In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.

Requirements:

• Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
• Minimum one year experience in an acute care setting, providing direct patient care; previous experience as a hospice LPN preferred.
