Job summary

TEEMA Solutions Group is seeking a Licensed Practical Nurse (LPN) to provide patient-centered nursing care under the direction of an RN or physician. The role includes administering medications, monitoring patient conditions, and documenting care in accordance with standards and regulatory requirements.

The successful candidate will have an accredited LPN program graduate status, an active state license, and a current BLS certification or policy-based alternative.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current Licensed Practical Nurse (LPN) license in the state of practice.
• Current BLS certification preferred or required per facility policy.
• Recent long-term care (LTC), skilled nursing (SNF), rehabilitation, or acute care experience preferred.
• Ability to maintain accurate clinical documentation and communicate effectively with patients and the healthcare team.

Responsibilities

• Provide direct nursing care and assist with the implementation of individualized care plans.
• Administer medications and treatments as prescribed.
• Monitor, assess, and document patient conditions, reporting changes to the appropriate healthcare provider.
• Assist with admissions, discharges, and ongoing patient care.
• Maintain accurate and timely clinical documentation.
• Collaborate with physicians, Registered Nurses, and the interdisciplinary healthcare team.
• Adhere to infection prevention, patient safety, HIPAA, and regulatory standards.
• Educate patients and families within the scope of practice.
