Job summary

TEEMA Solutions Group is seeking a Licensed Practical Nurse (LPN) to provide compassionate, patient-centered nursing care under the direction of an RN or physician. The LPN administers medications, monitors patient conditions, performs nursing procedures, documents care, and collaborates with the interdisciplinary team to promote positive patient outcomes.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current LPN license in the state of practice.
• BLS certification either preferred or required per facility policy.
• Recent experience in long-term care (LTC), skilled nursing (SNF), rehabilitation, clinic, or acute care preferred.

Responsibilities

• Provide direct nursing care and implement individualized care plans.
• Administer medications and treatments as prescribed.
• Monitor, assess, and document patient conditions, reporting changes to the RN or provider.
• Assist with admissions, transfers, discharges, and ongoing patient care.
• Maintain accurate and timely clinical documentation.
• Collaborate with physicians, RNs, and the interdisciplinary team.
• Follow infection prevention, patient safety, HIPAA, and regulatory standards.
• Educate patients and families within the scope of practice.
• Perform other duties as assigned.
