Job summary

Talented Medical Solutions is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to residents under the supervision of an RN or physician. You will administer medications, monitor conditions, document care, and collaborate with the interdisciplinary team to promote positive outcomes.

The ideal candidate has graduation from an accredited Practical Nursing program, an active LPN license in the state of practice, and familiarity with EMR systems.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Active LPN license in the practicing state.
• BLS certification preferred or required per policy.
• Long-term care experience preferred.

Responsibilities

• Provide direct nursing care to residents in accordance with individualized care plans.
• Administer prescribed medications, treatments, and therapeutic interventions safely and accurately.
• Monitor, assess, and document resident conditions, reporting significant changes to the RN or physician.
• Assist with admissions, transfers, and discharges.
• Perform wound care, catheter care, tube feeding assistance, and other nursing procedures within scope of practice.
• Collaborate with physicians, RNs, CNAs, therapists, and other members of the interdisciplinary healthcare team.
• Maintain accurate and timely electronic medical record documentation.
• Educate residents and families regarding medications, treatments, and healthcare needs.
• Follow infection prevention, patient safety, HIPAA, and facility policies.
• Respond appropriately to emergencies and changes in resident condition.
• Supervise and delegate appropriate tasks to nursing assistants as permitted by state regulations.
• Perform other job-related duties as assigned.
