Job summary

TEEMA Solutions Group is seeking a Licensed Practical Nurse (LPN) to provide compassionate, patient-centered care under the supervision of a Registered Nurse or physician. You will administer medications, monitor patient conditions, document care, and collaborate with the care team.

The ideal candidate will have an accredited LPN program graduate, an active LPN license in the state of practice, and strong communication skills.

Qualifications

  • Graduate from an accredited Practical Nursing program.
  • Current LPN license in the state of practice.
  • Current BLS certification preferred or required per facility policy.
  • Recent LTC/SNF/acute care experience preferred.
  • Ability to maintain accurate clinical documentation and communicate effectively.

Responsibilities

  • Provide direct nursing care and assist with individualized care plans.
  • Administer medications and treatments as prescribed.
  • Monitor and document patient conditions, reporting changes to the provider.
  • Assist with admissions, discharges, and ongoing patient care.
  • Maintain timely clinical documentation.
  • Collaborate with physicians, RNs, and the healthcare team.
  • Adhere to infection prevention, patient safety, HIPAA, and regulatory standards.
  • Educate patients and families within scope of practice.
  • Perform other duties as assigned.
