Job summary

Talented Medical Solutions is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to residents under supervision of an RN or physician. The LPN administers medications, monitors conditions, documents care, and collaborates with the interdisciplinary team to promote positive outcomes.

Responsibilities include admissions transfers discharges, wound and catheter care, education for residents and families, infection prevention, HIPAA compliance, and supervising nursing assistants

Qualifications

• Graduate of an accredited Practical Nursing program.

Responsibilities

• Provide direct nursing care to residents in accordance with individualized care plans.
• Administer prescribed medications, treatments, and therapeutic interventions safely and accurately.
• Monitor, assess, and document resident conditions, reporting significant changes to the RN or physician.
• Assist with admissions, transfers, and discharges.
• Perform wound care, catheter care, tube feeding assistance, and other nursing procedures within scope of practice.
• Collaborate with physicians, RNs, CNAs, therapists, and other members of the healthcare team.
• Maintain accurate electronic medical record documentation.
• Educate residents and families regarding medications, treatments, and healthcare needs.
• Follow infection prevention, patient safety, HIPAA, and facility policies.
• Respond to emergencies and changes in resident condition.
• Supervise and delegate tasks to nursing assistants as permitted by state regulations.
• Perform other job-related duties as assigned.
