Job summary

Talented Medical Solutions is seeking a Licensed Practical Nurse for a long-term care setting. You will deliver nursing care under the direction of a Registered Nurse or physician and supervise Certified Nursing Assistants on the unit.

The role requires LPN/LVN licensure in the state of practice, and BLS certification. You will maintain accurate documentation and communicate changes in resident condition to the care team. Prior LTACH experience is preferred.

Qualifications

• Graduate of an accredited practical or vocational nursing program
• Prior experience in a long-term care or skilled nursing setting is preferred
• LPN/LVN licensure in the state of practice is required
• BLS certification is required

Responsibilities

• Administer medications and treatments in accordance with physician orders and facility policy
• Perform resident assessments and monitor for changes in physical, cognitive or behavioral status
• Supervise and delegate appropriately to Certified Nursing Assistants on the assigned unit
• Complete accurate and timely documentation in the resident record and medication administration record
• Communicate changes in resident condition to the Registered Nurse, physician and family as appropriate
• Follow infection control, safety and regulatory requirements governing long-term care practice
