ContinueCARE Hospital at Baptist Health Paducah is seeking a dedicated Licensed Practical Nurse (LPN) to join our care team in a fast-paced hospital setting, delivering patient-centered nursing under supervision.

You will perform assessments, administer medications, document in the EHR, assist with admissions and discharges, provide family education, and collaborate with RNs and physicians to maintain safety, quality care, and compassionate service.

Qualifications

• Current, active LPN license in the State of Kentucky.
• Minimum of 1 year of recent hospital experience.
• Current BLS certification (American Heart Association or equivalent).
• Strong clinical skills and sound judgment.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced hospital environment.

Responsibilities

• Deliver direct patient care under the supervision of an RN or physician.
• Perform patient assessments and document observations and interventions accurately.
• Administer medications and treatments as prescribed.
• Monitor patient responses to treatments and report any changes to the RN or physician.
• Assist with admissions, discharges, and transfers.
• Collaborate with interdisciplinary teams to plan and implement patient care.
• Provide patient and family education and emotional support.
• Maintain accurate and timely documentation in the EHR.
• Uphold infection control and safety standards at all times.
