Job summary

HarborChase of Madison is seeking an experienced LPN/LVN to provide direct nursing care to residents under the supervision of the Director of Resident Care. You will ensure high-quality, safe health services and document care accurately, while administering medications and supporting resident self-care.

The role requires at least 1 year of nursing experience in a health care facility, valid state licensure, and training in dementia care.

Qualifications

• Graduate from an accredited LPN/LVN program and be licensed in the state of the community.
• Must obtain valid Continuing Education credits as required by the state.
• Training in Alzheimer and Dementia with focus on Validation Therapy.
• Minimum of one year nursing experience in a health care facility.
• Experience in a long term care community/geriatric nursing preferred.
• Able to communicate effectively with management, residents, families and external contacts.
• Experience with Microsoft Office and Outlook software preferred.

Responsibilities

• Ensures quality and safe delivery of health services to residents.
• Implements plan of care formulated by physician and DRC.
• Provides accurate and timely documentation of care given.
• Administers medication per physician orders and documents on MAR.
• Assembles resident charts and checks forms for completeness and accuracy.
• Prepares incident/accident reports and notifies physician and family as needed.
• Maintains safe and healthy working environment per clinical standards.
• Assists residents in learning self-care activities.
• Organizes resources for meeting residents' needs under care.
• Recognizes emergencies and initiates appropriate actions.
• Maintains confidentiality and resident rights.
• Performs other duties as assigned by supervisor.
