Job summary

HarborChase of Plano is seeking an LPN/LVN to provide direct nursing care under the supervision of the Director of Resident Care. You will administer medications, document on MAR, and coordinate plans of care with physicians and the DRC.

This role requires strong communication, safety awareness, and the ability to work with seniors in a long-term care setting. Texas licensure and Alzheimer care training are preferred.

Qualifications

• Graduated from an accredited LPN/LVN program and currently licensed in the state.
• Must obtain valid continuing education credits as required by the state.
• Training in Alzheimer and Dementia with Validation Therapy focus.
• Minimum of one year nursing experience in a health care facility.
• Long-term care or geriatric facility experience preferred.
• Experience in emergency care and first-aid.
• Able to communicate effectively with all levels of management, associates, residents, and families.
• Experience with Microsoft Office and Outlook preferred.

Responsibilities

• Provides direct and individual nursing care to assigned residents.
• Administers medications per physician orders and documents on MAR.
• Assesses resident and family information for diagnosis or plan of care.
• Maintains accurate documentation of care given.
• Organizes resident charts and ensures forms are complete and accurate.
• Ensures safe, compliant operation and confidentiality of resident information.
