Job summary
Evergreen Center for Health & Rehabilitation is seeking a Licensed Practical Nurse to provide direct nursing care to residents at our Stafford Springs location. You will administer medications, monitor health status, and collaborate with a multidisciplinary team to implement individualized care plans.

Qualifications
- Valid state LPN license.
- Compassionate and empathetic approach to patient care.
- Ability to work with aged and chronically ill patients.
- Strong clinical nursing skills including medication administration.
- Excellent communication and interpersonal abilities.
- Ability to work collaboratively in a multidisciplinary team.
Responsibilities
- Administering medications and treatments as prescribed by healthcare providers.
- Monitoring and recording patient vital signs and medical information.
- Collaborating with the interdisciplinary team to develop and implement care plans.
- Providing wound care and other nursing interventions as needed.
- Supervising and supporting CNAs in delivering care.
- Ensuring compliance with all regulatory standards and protocols.
