Job summary

Evergreen Center for Health & Rehabilitation in Stafford Springs, CT, is seeking a Licensed Practical Nurse to provide direct nursing care, administer medications, and support residents and their families. You will join a caring team delivering person-centered care.

You will collaborate with an interdisciplinary staff, monitor vital signs, and help implement care plans while ensuring regulatory compliance. A valid LPN license is required, with opportunities for professional growth and mentorship

Qualifications

• Valid state license (LPN) is required.
• Compassionate and empathetic approach to patient care.
• Interest in nursing needs of the aged and chronically ill; ability to work with both.
• Strong clinical skills including administration of medications.
• Excellent communication and interpersonal abilities.
• Ability to work collaboratively in a multidisciplinary team.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor and record patient vital signs and medical information.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Provide wound care and other nursing interventions as needed.
• Supervise and support CNAs in delivering care.
• Ensure compliance with regulatory standards and protocols.
