Job summary

Qualifications
- Active Florida LPN license
- 1–2 years of LPN experience in sub-acute or skilled nursing
- Strong communication and interpersonal skills
- Ability to work in a fast-paced environment
- Attention to detail and organizational skills
- Commitment to high-quality patient care and safety
Responsibilities
- Provide direct patient care: medications, vital signs, and routine treatments
- Develop and implement individualized care plans with the interdisciplinary team
- Monitor and report changes in patient condition to RN or physician
- Maintain accurate patient care records
- Participate in in-services and training to stay current with best practices
- Collaborate with healthcare professionals for seamless care coordination
