Job summary

Wellpath seeks a Licensed Practical Nurse (LPN) in Pennsylvania to support patient care under RN supervision in our health care units and housing settings. You will participate in assessment, planning, and education, administer medications, and coordinate care with the health team.

Qualifications

• Graduate from an accredited school of nursing.
• Minimum of one year clinic experience preferred.
• Current licensure as a Licensed Practical/Licensed Vocational Nurse in the state of employment.
• CPR certification required.

Responsibilities

• Under RN supervision, assesses patients and contributes to individualized treatment planning.
• Administers medications, including IV therapy (if certified), and performs diagnostic tests per protocols.
• Collaborates in controlled substance counts and adheres to safety procedures during shifts.
• Assists in procedures, implements nutrition and therapeutic plans, and documents encounters using SOAP format.
• Attends mandatory staff meetings and training to ensure high-quality patient care.

Education

Graduate from an accredited school of nursing.
