Wellpath in Kentucky is seeking a Licensed Practical Nurse (LPN) to join a dedicated care team providing compassionate, patient-centered nursing services in our health care units and housing environments.

Under RN supervision, you will administer medications, participate in care planning, and support clinical staff with documentation and patient education. A current LPN license and CPR certification are required, with new opportunities for professional growth.

Qualifications

• Graduate from an accredited school of nursing.
• Current LPN/LVN license in the state of employment and CPR certification.
• Prefer at least 1 year of clinic experience.

Responsibilities

• Under RN supervision, administers medications and IV therapy if certified.
• Collaborates in care planning and documents nursing encounters.
• Performs patient assessments and supports routine nursing care.
• Communicates with patients, families, and team members.
• Attends staff meetings and training to uphold high-quality care.
