Job summary

North Suffolk Community Services, Inc. is seeking an LPN in Winthrop, MA to oversee health care services and ensure regulatory compliance for residents. The role includes supervising CNAs, coordinating follow-ups, and providing medical training to staff.

The LPN will participate in health care planning, manage treatment plan approvals, and conduct annual MAP audits while adhering to DDS regulations and MAP policies.

Qualifications

• Education/Licensure: Licensed Practical Nurse, registered with the MA Board of Registration in Nursing.
• Experience: Minimum of one year of experience in a community health setting.
• Skills: MAP certification within five months; First Aid/CPR certification within five months of hire.

Responsibilities

• Assist with health care services provided to individuals supported; participate in medical discharge meetings at hospitals.
• Obtain reports and approval of treatment plans from PCP/Specialists.
• Review seizure and medical incident reports and make recommendations to program staff and assist with follow up as necessary.
• Participate in managers', clinical and staff meetings to review health care needs and global health care planning with all staff.
• Provide medical training in Diabetes, Nutrition, Seizures, Oxygen and Vital Sign training to staff.
• Conduct MAP audits at site yearly and follow all MAP policies and procedures.
• Perform direct support related tasks in emergencies or as requested by supervisor.
• All other duties assigned by supervisor.
