Job summary

River Valley Family Health Centers is looking for an experienced Licensed Practical Nurse (LPN) to join our clinics as a Clinical Care Coordinator in Montrose and Delta Colorado. This full-time role covers 40 hours per week with a pay range of $27.00–$34.00 per hour and a comprehensive benefits package.

The LPN will perform routine clinical procedures, provide medical triage, educate clients, and collaborate with staff to coordinate patient care.

Qualifications

• High School Diploma or equivalent.
• Completion of an approved LPN program and active Colorado licensure.
• CPR/BLS certification required.
• IV certification preferred.
• Minimum of 2 years in direct patient care in a clinical or ambulatory setting.
• Excellent verbal and written communication skills and ability to work with diverse populations.
• Ability to work in a fast-paced environment with minimal supervision.

Responsibilities

• Perform routine clinical tasks: vitals, CLIA-waived labs, patient history and chief complaint.
• Provide medical triage and obtain direction from the provider as needed.
• Educate patients and families on health-related topics.
• Collaborate with nursing staff and providers to coordinate care.
• Travel to different clinic locations as needed.
