Great Lakes Management Co. is seeking an LPN (Licensed Practical Nurse) for a full-time role in Colorado. The position offers an hourly range of $32.00 to $38.00 with a 2 days per week schedule and potential rotating weekends. LPN certification within Colorado is required.

Qualifications

• Maintains a current state license as a LPN per state regulations.
• A minimum of one year's experience in home health, assisted living or long-term care is preferred.
• Good organizational skills with the ability to self-start and follow through with initiatives with minimal supervision.
• Exhibits a caring heart with a passion to care for the elderly.

Responsibilities

• Ensures residents meet admission criteria, monitors progress, and recommends placement if necessary.
• Keeps residents' care plans up to date per state regulations.
• Monitors and maintains resident charts and progress notes.
• Communicates changes in residents' needs to physicians, staff and family.
• Coordinates resident services with other providers.
• Oversees medications and follow-ups with physicians as needed.
• Ensures medication administration procedures are followed correctly.
