Job summary

River Valley Home Health and Hospice in Colorado River Valley is seeking an licensed practical nurse to provide direct patient care in home health settings. You will implement care plans initiated by the registered nurse and document accurately to reflect the plan of care.

The role requires strong communication, reliable transportation for community visits, CPR certification, and a commitment to teamwork with RNs, physicians, and other disciplines.

Qualifications

• Must have completed a practical nursing program or be licensed in the state.
• Three years nursing experience preferred.
• Good verbal and written communication and organization skills.

Responsibilities

• Provide direct patient care per Nurse Practice Act.
• Implement care plan initiated by the registered nurse.
• Document accurately and timely the plan of care.
• Coordinate with RN(s) and physician(s) for patient needs.
• Drive/transport for agency-related visits and duties.
