Piedmont Health Senior Care is seeking a Clinic Nurse (LPN) for the PACE Nursing team in Pittsboro, NC. The role supports comprehensive, coordinated care under the PACE model while following the NC Nurse Practice Act and organizational policies.

Work location is 163 Chatham Business Dr, Pittsboro, NC 27312, with a Monday–Friday, 8:00am–5:00pm schedule. Requires NC LPN license, BCLS, and ambulatory care experience; bilingual is a plus. Benefits include medical, dental, vision, life, and a 403(b).

Qualifications

• Practical Nursing Accredited Program completed and Licensed Practical Nurse (LPN) certification.
• Current and valid NC LPN license; BCLS certification.
• 1 year of LPN experience preferred within last 3 years.
• Ambulatory health care experience preferred; bilingual a plus.

Responsibilities

• Uses nursing process for assessment, planning, implementation, evaluation and education.
• Supports providers by anticipating needs and assisting in care delivery.
• Coordinates services with health-care team and external partners for complex patient care.
• Performs triage via onsite and telephone communication and assists with check-in when needed.

About Position:

Duties/ Responsibilities -Utilizes the nursing process including: assessment, planning, implementation evaluation and recording of patient care and patient education. Provides support to providers by anticipating needs and providing assistance in the provision of care. Coordinates services with other members of the health-care team, internal and external, to provide complex patient care. Answers or returns phone calls. Provides onsite and telephone triage. Assists with the check in process when needed. Serves as a member of the interdisciplinary care team and participates in huddles. Performs other duties as assigned. Practices within the scope of the North Carolina. Nurse Practice Act and Piedmont Health Services’ policies, procedures, and standards. Performs all other duties, as assigned.

Qualifications -

• Education: Practical Nursing Accredited Program, License Practical Nurse Diploma or Certificate
• Licensure, Registry or Certification Required: Current & Valid NC License Practical Nurse; Basic Cardiac Life Support (BCLS).
• Experience: One year of general Licensed Practical Nurse experience preferably within the last three years.
• Preferred experience: Previous experience in an ambulatory health care setting; bi-lingual

Immunizations:Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.

Pay Range: $59,050 - $79,358(commensurate with years of experience)
