Job summary

Vitality Living is hiring an LPN Charge Nurse for the Stony Brook Community in Louisville, KY. You will administer or assist with medications, coordinate care, and work with the Wellness Director to update service plans, while ensuring accurate charting and effective communication with physicians.

Ideal candidates are 21+, fluent in English, with an active LPN license and CPR certification. Vitality Living offers a supportive team environment and opportunities to grow within our vibrant

Qualifications

• Must be at least 21 years old.
• Demonstrate ability to communicate effectively in English, both verbally and in writing.
• Meet state health related requirements.
• Maintain current professional license and CPR certification if currently LPN/LVN.
• Maintain any other certifications as required by state or provincial regulations.

Responsibilities

• Administrate or assist residents to self-administer medications and treatments
• Collect, store, and ensure transport of certain specimens for laboratory studies
• Communicate with physicians via telephone, fax, and Physician Communication forms
• Ensure physician orders, laboratory reports, and other data is scanned and filed in resident charts
• Assist in the development and revision of service plans. Inform Wellness Director of current resident needs and preferences as well as successful service approaches
• Perform other duties as directed by Wellness Director
