Southland Health & Rehab in Peachtree City, GA is seeking a Licensed Practical Nurse to join our mission-driven team. You will work with RNs, CNAs, and an interdisciplinary care team to deliver compassionate, high-quality resident care in a supportive, servant-leadership culture.

Qualifications

• Active Licensed Practical Nurse (LPN) license (State of Georgia or eligible).

• Strong clinical judgment and critical thinking skills.
• Excellent communication and teamwork abilities.
• Reliability and a commitment to excellence.
• Prior long-term care experience preferred, but not required.

Responsibilities

• Administer medications and treatments as prescribed
• Monitor residents' conditions and report changes to the care team
• Provide direct nursing care in accordance with care plans
• Supervise and support CNAs in delivering quality care
• Maintain accurate and timely documentation
• Ensure compliance with all clinical policies and regulatory standards
• Communicate effectively with residents, families, and team members
